Acceptance of Terms

By accessing or using primetestosterone.com (the “Site”) or any service offered by Prime Testosterone Men's Health PLLC (“we,” “us,” or “our”), you agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree, please do not use the Site or our services.

Eligibility

No Doctor-Patient Relationship via Website

Browsing the Site, contacting us through online forms, or scheduling an introductory call does not, by itself, create a provider-patient relationship. A clinical relationship is established only after a completed intake, a clinical evaluation, and formal acceptance into our practice.

Membership & Payment

Testosterone replacement therapy is offered on a recurring subscription/membership basis. Additional services — including medical weight loss, peptide therapies, and certain lab panels — are offered on a cash-pay basis. Current pricing, billing intervals, included services, and cancellation terms are disclosed in writing at the time of enrollment.

You are responsible for keeping a valid payment method on file. Failed payments may result in a pause or termination of services. Refunds, when issued, follow the policies provided at enrollment.

Cancellation

You may cancel your membership at any time by contacting our office. Cancellations take effect at the end of the current billing cycle, and we do not pro-rate partial months unless required by law. Active prescriptions may be discontinued upon cancellation.

Acceptable Use

You agree not to misuse the Site by attempting to gain unauthorized access, interfering with its operation, scraping content, transmitting malware, or using it for any unlawful purpose. We reserve the right to suspend or terminate access for any user who violates these Terms.

Intellectual Property

All content on the Site — including text, graphics, logos, photographs, and designs — is owned by or licensed to Prime Testosterone Men's Health PLLC and is protected by copyright, trademark, and other intellectual property laws. Governing Law

These Terms are governed by the laws of the State of Oklahoma, without regard to its conflict of law principles. Any dispute arising out of or related to these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Marshall County, Oklahoma.
