HIPAA Notice of Privacy Practices

Our Commitment

Prime Testosterone Men's Health PLLC is committed to protecting the privacy and security of your Protected Health Information (PHI). We are required by federal law (the Health Insurance Portability and Accountability Act, or HIPAA) to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.

This Notice describes how medical information about you may be used and disclosed and how you can get access to that information. Please review it carefully.

How We May Use Your Information

Treatment: We use and share your PHI to provide you with medical care and to coordinate treatment with other providers, labs, and pharmacies involved in your care.

Payment: We use and share your PHI to bill and collect payment for the services we provide — including processing membership payments, verifying coverage, and submitting claims when applicable.

Healthcare Operations: We use and share your PHI to operate our practice — including quality improvement, staff training, business management, compliance reviews, and similar administrative activities.

Required by Law: We may disclose PHI when required by federal, state, or local law — for example, to public health authorities for disease reporting, to report suspected abuse or neglect, in response to court orders or subpoenas, or to law enforcement under specific legal circumstances.

Uses Requiring Your Authorization

Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of PHI require your written authorization. You may revoke an authorization at any time, in writing, except to the extent we have already acted in reliance on it.

Your Rights

You have the right to inspect and obtain a copy of your medical and billing records, generally within 30 days of a written request.

You have the right to request amendments to PHI you believe is incorrect or incomplete. We may deny a request under certain circumstances and will provide a written explanation.

You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requested restrictions, but we will accommodate reasonable requests when possible.

You have the right to request that we communicate with you about medical matters in a specific way or at a specific location (for example, by mobile phone only, or to an alternate address).

You have the right to receive an accounting of certain disclosures of your PHI made in the six years prior to your request.

You have the right to receive a paper copy of this Notice upon request, even if you have agreed to receive it electronically.

Breach Notification

In the unlikely event of a breach of unsecured PHI, we will notify you in accordance with HIPAA's Breach Notification Rule.

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint directly with us by emailing info@primetestosteronenow.com or calling (580) 289-7430. You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against in any way for filing a complaint.

Changes to This Notice

We reserve the right to change the terms of this Notice and to make the new terms effective for all PHI we maintain. Updated Notices will be posted on this page and made available in our office.
